The measurement of testosterone and androstenedione in female patients is usually performed to assess androgen excess which can cause several hormonal disorders. There are many causes of androgen excess mostly related to endocrine and iatrogenic causes. Historically, mostly immunoassays have been used to measure these analytes. However, immunoassays, particularly for testosterone, are prone to interference from related steroids and steroid like compounds. This study describes the development and validation of an automated SPE-LC-MS/MS method which offered higher sensitivity and selectivity compared to the immunoassays. The fully automated sample preparation features internal standard and reagent addition, protein precipitation and centrifugation, coupled directly to LC-MS/MS. The system is capable of handling 98 samples in 13 hours and 15 minutes.
